This is a very important section. As per Ofsted regulations, we are required to explore full education and employment history from age 11. Any periods not working/gaps in employment must be explained.
If the role is via an employment agency, please provide details of the agency, rather than the company. This is required to ensure references are requested from the correct companies.
Please provide a full postal address for any company you have worked for with children or vulnerable people. It is also required by Ofsted that we explore and confirm the reason for leaving those posts.

Where an applicant has lived abroad within the last 5 years, it is requirement that they provide a Certificate of Good Conduct (or similar) from each Country they lived in.
The post for which you are applying is covered by the Disqualification from Caring for Children (England) Regulations 2002 and Disqualification from Caring for Children (Wales) Regulations 2004. You are therefore required to complete this form before you can be considered for this post.

Please give full details for 3 mandatory referees. We require two professional referees who will be able to comment on your ability to perform the job for which you are applying. The first should be your current or most recent employer, the second a previous employer. One of your professional referees must be able to comment on your suitability for the post for which you are applying.
Please note that we may also take up references in respect of any previous employers from those provided without further notice from you.
A third reference is sought from a personal referee. Please note that references from your spouse, partner or family members are not acceptable.
